What is the difference between a drug and a medicine (is there a biological distinction or is it just linguistic?)
It''s a semantic difference. "Drug" is a generic term for something that has a biological effect when administered. This is of course very broad and covers such things as narcotics and poisons. A medicine or medication is a drug which is prescribed for a medical purpose by a physician.
